Procedure to apply for replacement of damaged passport
As far as possible keep all the details same as in the old passport like address, name, etc. If you change anything, you need a proof for that. (in case of change in address - Annexure I)
1. Application Form 1
2. An affidavit stating how the passport got damaged, blah blah
3. Pan card, Voter id card, driving license, old passport, marks card, degree certificate all 2 copies self attested with originals.
4. A letter to Regional Passport officer requesting as to why you need passport under tatkaal scheme.
5. 6 photographs just in case ( 3 - 4 actually needed).
Finally go there very early in the mornint 7 - 7:30 and sit in the line, coz only 300 passport cases will be handled per day.
The Enquiry counter guy knows nothing about what documents to submit or procedure to follow, so don't ask in enquiry counter instead ask the ppl who help filling the form for illiterates and the tea shop guy or the xerox shops around that office.
